The Supported Hostpital Exit Program (previously known as Outreach) works to reduce the risks associated with discharge from psychiatric care by facilitating an organised and focused approach to hospital recovery. Taking place over a period of three months, the SHEP program begins while clients are still in hospital. At this point we are able to help the patient indentify issues that may hinder their recovery and develop goals and coping strategies that may help them cope in the home environment. Once the patient is discharged the SHEP program is designed help them realise the goals they identified in hospital by providing one on one support.
